Cashless Physiotherapy Singapore: How Direct Billing Works at BMJ

BMJ physiotherapist in navy scrubs discussing cashless physiotherapy with a patient in Singapore.

Cashless physiotherapy in Singapore can reduce the amount you need to pay upfront for eligible treatment. With direct billing, the clinic bills the approved portion to your insurer or benefits administrator. Your policy, approval requirements and any patient contribution still apply.

Perhaps you are planning rehabilitation after knee surgery. Or a sports injury has made everyday movement uncomfortable. Alongside choosing a physiotherapist, you may be wondering how to use your insurance benefits without handling every claim yourself.

At BMJ Physiotherapy, direct billing may be available for eligible patients under supported arrangements. Checking before your appointment helps you understand what can be billed directly and what you may need to pay.

What is cashless physiotherapy?

Cashless physiotherapy is a payment arrangement in which approved treatment charges are billed directly to an insurer or benefits administrator. You settle any amount that falls outside the approved cover.

This can reduce the need to pay the full treatment fee first and claim it back later. However, cashless physiotherapy can still involve co-payments, deductibles or charges above your benefit limit.

Two separate checks matter: whether your policy covers the treatment and whether direct billing is available for that treatment at the clinic you choose.

An insurance membership card helps identify your plan. It does not, by itself, confirm payment approval. Cigna’s member FAQs, for example, explain this distinction.

How does direct billing differ from paying and claiming?

The main difference is who pays the treatment fee first. Both arrangements remain subject to your policy’s benefits and claims rules.

Question Direct billing Pay and claim
Who pays first? The approved portion is billed directly. You pay any patient share. You pay the clinic, then request reimbursement.
Who handles the submission? The clinic submits charges under the agreed arrangement; you may need to provide documents. You submit the required claim documents to your insurer or administrator.
What determines the amount covered? Policy terms and the scope of approval. Policy terms and the insurer’s claims assessment.

For example, two colleagues could attend the same clinic with different employer plans. One might have direct billing, while the other needs to pay and claim.

Ask about your specific plan rather than relying on another person’s experience.

Can you use direct billing at BMJ Physiotherapy?

Cashless physiotherapy at BMJ depends on your individual plan, the treatment required and the billing arrangements available for your appointment.

Before booking cashless physiotherapy, clarify:

  • Whether your plan includes outpatient physiotherapy.
  • Whether BMJ can bill your insurer or benefits administrator directly.
  • Whether you need a doctor’s referral or approval before treatment.
  • Whether session limits, financial limits or patient contributions apply.

An employer’s benefits administrator may manage access to treatment and claims. This organisation is sometimes called a third-party administrator, or TPA. Its name may appear on your membership card or benefits app.

For employer benefits, your HR team can help identify the correct plan or administrator.

How do you arrange cashless physiotherapy at BMJ?

Four steps for cashless physiotherapy at BMJ: contact the clinic, check requirements, confirm payment and attend with the requested documents.
Preparing for cashless physiotherapy? Check your policy requirements, approval and patient contribution with BMJ before your visit.

Contact BMJ before your first appointment and explain that you would like to arrange cashless physiotherapy. This gives the team an opportunity to clarify the next steps for your plan.

1. Contact your preferred clinic

Tell the team which clinic you wish to attend and the name of your insurer or benefits administrator. Give a brief description of why you need physiotherapy.

You can find current branch contacts on our clinic locations page.

2. Check the approval requirements

Pre-authorisation means obtaining an insurer’s approval before treatment. Depending on the arrangement, the insurer may need information about your condition, proposed care or treating provider.

Some arrangements use a Letter of Guarantee, or LOG, to set out approved payment terms. Others use an authorisation code or another form of confirmation.

Bupa Global’s pre-authorisation guidance explains how advance approval can support direct settlement with network providers. Requirements differ between plans.

3. Confirm the approved scope

Before attending, ask what the confirmation covers. Useful details include the treatment, clinic, approved dates, session allowance and any amount payable by you.

Allow time for any additional documents requested. Ask for the expected response time for your case rather than assuming approval will be immediate.

4. Attend and check any remaining balance

Bring the documents requested for your appointment. Settle any agreed patient contribution and retain the billing records provided.

If further sessions are recommended, check whether your existing approval covers them or whether another request is needed.

What documents might you need?

Cashless physiotherapy checklist: insurance details, identification, referral, medical documents, approval or LOG, and insurer forms, as required.
Your insurance plan determines which documents you need. Check with BMJ before your visit, including whether a referral or Letter of Guarantee is required.

The documents required for cashless physiotherapy depend on your plan and approval process. Having the relevant information ready can reduce avoidable follow-up.

You may be asked for:

  • Your insurance membership card or digital membership details.
  • Identification to verify the patient’s details.
  • A doctor’s referral, where required.
  • Relevant medical reports or a hospital discharge summary.
  • Existing authorisation details or a Letter of Guarantee.
  • Any forms requested by the insurer or administrator.

A referral requirement for insurance is separate from being able to book a physiotherapy appointment directly.

Check the rules before starting treatment, including whether the referral must be obtained beforehand. Our article on doctor’s referrals for physiotherapy insurance claims explains this further.

Ask the clinic which documents are needed and how to provide them.

Will you still need to pay anything?

You may need to pay part of the bill even when cashless physiotherapy is approved.

A co-payment or co-insurance is your share of an eligible charge. A deductible is an amount you must pay before the relevant insurance benefit pays out. Benefit limits and exclusions can also affect your final balance.

MoneySense’s guide to health insurance terms explains these costs in plain language.

An illustrative example

Suppose a session costs S$150, and the approved arrangement pays 80% of the eligible fee. Assume there is no deductible, fee cap or excluded charge affecting this example.

The insurer’s portion would be S$120, and the patient would pay S$30.

These figures are illustrative. They are not BMJ’s published fees or a statement about any particular policy.

Ask about your expected contribution before treatment so you can plan for it.

What happens if approval is pending or direct billing is unavailable?

If you are arranging cashless physiotherapy, discuss payment with the clinic before treatment begins. You may need to pay first, even if you expect to submit an insurance claim later.

If direct billing is unavailable, ask your insurer whether reimbursement is possible and what documents it requires. Keep the itemised invoice, receipt and any supporting documents requested.

If approval is pending, clarify who will pay for the appointment and whether treatment received before approval would be eligible for reimbursement.

Starting treatment does not confirm that the insurer will pay afterwards. Obtain the relevant payment information before deciding how to proceed.

How can BMJ support your treatment and claims paperwork?

BMJ can provide appropriate treatment documentation to support an insurance submission, such as invoices and relevant clinical records or reports. The documents needed depend on the claim.

Our physiotherapists provide services including sports injury rehabilitation and post-surgery rehabilitation. Your physiotherapist will discuss care based on your condition and rehabilitation needs.

Your treatment recommendation and insurance allowance serve different purposes. An insurer’s approved session limit sets the payment allowance under that approval; your clinician can explain the care recommended for your recovery.

Whether you are considering Tampines, Marine Parade, Kovan, Ang Mo Kio, Kallang or One@KentRidge, tell the team your preferred location when asking about cashless physiotherapy. Confirm the arrangement for that appointment.

BMJ can assist with relevant paperwork. Your insurer or administrator decides eligibility and payment under your plan.

Frequently asked questions

Can I arrange cashless physiotherapy without a doctor’s referral?

This depends on your plan. Some insurance arrangements require a referral before treatment or approval. Check the requirement even if you can book a physiotherapy appointment directly.

Can employer medical benefits include direct billing?

Cashless physiotherapy may be available under some employer outpatient benefits. Ask your HR team or benefits administrator about your specific benefits, approved providers and any referral requirements.

Does approval cover every physiotherapy session?

Read the approval details. Authorisation may apply to a specified number of sessions, treatment period or financial allowance. Check the remaining allowance and any further approval needed before continuing under direct billing.

How long does approval take?

Timing depends on the insurer, the information required and whether further review is needed. Contact BMJ ahead of your appointment and ask about the process for your case.

What if my insurer’s name appears on a clinic’s partner list?

Ask the clinic to check your exact plan and appointment details. An insurer can offer several plans with different benefits, networks and billing rules.

Can I claim physiotherapy after surgery or a sports injury?

It may be eligible under your policy. The reason for treatment, referral requirements, benefit limits and supporting documents can affect the decision. Check your cover before assuming rehabilitation costs will be reimbursed.
